The Thing
Exorcist
Halloween (1978)
The Texas Chainsaw Massacre (the old one, I forgot which year it was made)
28 Days Later
Romero's Dead series, alway a classic!
Dawn of the Dead (the remake)
That's more of a thriller though.
Very good nonetheless.
try a film called "Begotten." it's somewhat hard to find, but i can almost guarantee that it will freak you out.
You want truly disturbing? Then you need to see the end all films that is Salo or 120 Days of Sodom.
Yea, Salo or the 120 Days of Sodom, is truly fucked up. Haven't seen Irreversible, want to, heard that also fucked up.

I freakin love Begotten. Have a burned copy since I can't find a good DVD for under $100. Sure it's like 60 minutes with no dialogue and half the time you couldn't make out what you were watching, but I never got bored or could take my eyes off the TV.

Other movies that are messed up but not as disturbing or Alejandro Jodorwoski's El Topo and Holy Mountain.
Henry: Portrait of a Serial Killer was shocking in it's realism and I highly recommend it

Anyways here are a few more since I decided to browse through my movie collection

Last House On The Left (1972)
The Man from the Deep River
Cube series
Street Trash (for laughs)
Nekromantik 1 & 2
All GR8 FILMS but what about The Devil's chair, The Signal, 30 days of night , nightmare on elm street collection and a cheap film called The Slaughter witch can be bit of a comadie
any zombie movies. 30 days of night rocked but the ending was kinda lame. cujo is a classic cuz is is realistic
I love Stephen King movies.
Yeah they're not completely terrifying but they do keep you on the edge of your seat...like Rose Red, or Kingdom Hospital for instance.
